Roger,

For what it is worth . .. .  attached find two JPEGs showing the location of 
the high field condition that develops when the gate to drain voltage of a 
MOSFET is too high. The situation begins to occur beyond pinchoff (e.g., when 
the reset MOSFET turns off). The electric field generated causes all kind of 
problems. In order as the drain voltage increases .. . 

1). Shot noise. Seen in the read noise of the output MOSFET a few volts 
beyond pinchoff

2). Luminescence. Glowing amplifier. 

3). Hot electron oxide charging. Flat-band shift near the gate drain edge.

4). Destruction. CCD stops working

The problems apply to both the reset and output amplifier MOSFETs.

Anyway . . you might find the plots interesting independent of your problem. 
The MOSFET has a universe of its own.
